It's about not being able to hide the true feelings while one is intoxicated. Even though he might keep this girl out of his head and avoid his feelings while sober, when he is intoxicated he only wants her.

I think the song is about a person liking another person and them and have strong feelings about and they constantly try and try but when they want then is when they are under the influence of drugs or anything like that so the feelings aren't reciprocated.

I think this song is about being heartbroken and a person only wanting you when they are intoxicated or using you for your body.

Basically saying why do you only want me when you are high. The guy obviously hides his true feelings from her when he's not intoxicated in some way I guess.
