The singer is so infatuated with this girl that he can't produce the will to live without her. They get in a fight (finger on the trigger to my dear Juliet) and she ends up dying. He can't live with the guilt so he purposely crashes his car, instantly killing himself so he can be with her again.

He is in love with someone he can't be with and the lyric "out from the window see her backdrop silhouette" shows that Juliet is dead. Now he feels that it's his fault and he can't live with the guilt of never reaching out. "I can't take this anymore" shows that he can't continue living with the pain of Juliet dead, so he's going to take his own life. Lastly, crashing his car through her window means that he has taken his life via car accident. Reach out to those you feel untraceable because you never know how close they might be to dying.
