I think that this song is about a “relationship” with someone, where one person feels so much emotion for the other, however the other person doesn’t even know their “name” (which could be taken literally, or name could be a metaphor for personality, their real self, etc). “And i would die or stay/ for you right now” suggests a one sided love (or an unrequited love).
“I’m dreaming of all of the possibilities”/ “And everytime i think the plan is aligning/ you’re still so close yet so far”
implies that the “possibilities” of the other person loving them back is “so close yet so far”.
