Well I think this one is quite obvious. It's from the perspective of someone who, ever since they were young, wanted to be a doctor to save the sick. But when they become a doctor, they realize that the entire American healthcare system is corrupt and greedy, and this song also remains true to Penelope Scott's often anti-capitalist lyrics, like Rät. Once they see how the system isn't working for the people, they don't how to feel about it, and they start feeling immoral and they don't know what to do about it. You can see it in the lyrics,

I wouldn't have it any other way,
I just think that they should feel good while they are alive." and,
"You corporate fucking prick
I did not become a doctor just to suck the devil's dick."

We all know Penelope Scott's lyrics choice is never a coincidence, it's certainly about someone wanting to be a doctor that's for sure. And I believe this song is about how greedy & sick the American healthcare system tends to be.
"Well, people die every day, I wouldn't have it any other way, I just think that they should feel good while they are alive." and "You corporate fucking prick. I did not become a doctor just to suck the devil's dick."
Shows this perfectly. It shows that--maybe when she was younger--she wanted to be a doctor, but she noticed how corrupted the American healthcare system is as she got older. And with that said, she probably felt unnerved that a healthcare system, made for caring people, not collecting their money, would be so greedy.

When I first heard this song, I thought of it as someone who became a doctor so they could save lives and improve the lives of others. They came to the realization, however, that while they can improve the physical condition of people by becoming a doctor, the lives of so many people were still subject to the dangers and horrors of today’s world. Making it in the real word is difficult, and the financial difficulties of medical bills only adds to that, so even though doctors are still saving lives and that is a very good thing, that does not provide complete relief from the hardship in the world.

About a doctor growing up and hoping to help folks get better but then understands that the healthcare folks are a bit greedy sometimes and they are like "I DIDNT BECOME A DOCTOR FOR THIS?!?!?"
