Peter Gabriel has long been an activist for human rights around the world, and I think this song (which was written in the early Eighties, while the Soviet Union still existed) was written in support of Soviet dissidents, who were often punished by being placed in psychiatric institutions.
Russians who protested against the Communist government's actions were often deemed insane (since, in theory, the Soviet Union was a Utopia,. only a crazy person could oppose it). These people were sane, but were placed in asylums and treated by medical doctors who surely had to KNOW the situation was wrong... but those doctors ignored their Hippocratic Oaths and followed orders from the state.
Peter Gabriel was encouraging political prisoners in asylums to remain strong, never to give in, and to remember that the outside world appreciated their courage.
